For my mom, "help" sometimes meant something as simple as going to get her a glass of water or getting something out of the car for her because her knees were hurting and stiff and she wouldn't have to walk across the house/to the garage. I think your wife is lucky to have a husband who's understanding and helpful, I'll tell you that. It's hard to someone with health problems, both to accept how it limits your life as well as theirs, and also to have to them constantly hurting/miserable. Is hiring someone to help out out of the question, financially? What about paying a friend or relative (who would likely charge less, or you could possibly trade them for some service you or your wife could offer)? Is there any sort of work your wife could do say, crafts she could sell, some sort of writing or editing that she could do over the 'net that might bring in a bit of money that could go toward paying for housekeeping help?
